Authorities in Ottawa County are investigating a fatal crash from Monday afternoon.

Troopers say it happened along SR-2 near Leutz Road around 3:30 p.m.

Investigators are now trying to determine why 48-year-old Wesley Washington drove his vehicle off the side of the road, then overcorrected, and hit an on coming minivan.

Washington was pronounced dead at the scene.

The driver of the mini-van was taken by lifeflight to Mercy St. Vincent's.
