Toledo nursing home faces license revocation

TOLEDO -- The Ohio Department of Health has notified the Liberty Nursing Center of Toledo on Ashland Avenue that its license faces revocation. The Department cited two instances, one in July and one in November in which residents at the facility may have been the victim of sexual misconduct and that in one instance staff "failed to immediately report the allegation of sexual abuse to the administrative staff."

The nursing home has 30 days to appeal the revocation noctice of November 23.  Management of Liberty Nursing Center declined to comment when contacted by WNWO.
